Fifth-seeded Russian Medvedev was up 6-1, 5-0 in the second round match when Serbian Kecmanovic declared he was out.
With 55 minutes remaining in the second set, Kecmanovic, 24, lost his serve for the third time and shook his head at the chair umpire.
The winner of the 2021 US Open, Daniil Medvedev, will next face either Czech Tomas Machac or Argentinean Mariano Navone, the 31st seed.
