The 2021-launched Sergei Kotov is said to have been struck early on Tuesday morning.
The Black Sea navy ship sustained damage to its right and left sides in addition to its stern, according to the Ukrainian military intelligence service.
The Kremlin has not yet responded.
Andrii Yusov, a Ukrainian intelligence official, informed the news television that the strike had left the servicemen on board dead or injured.
Project 22160, a patrol ship program for the Russian Navy, has produced four ships, including the Sergei Kotov.
The ship was involved in the assault on Snake Island on the first day of the full-scale invasion of Ukraine, according to Ukraine.
