On July 5, 2015, Lamara Bell, 25, and John Yuill, 28, lost their lives when their car careered off the M9 near Stirling when they were returning from a camping trip.
Despite the police being notified of the event, they were found dead on July 8 after lying in their Renault Clio for three days.
Mr. Yuill, a father of five, was declared dead at the scene, and Ms. Bell, a mother of two, passed away in a hospital four days later.
Following a fatal accident inquiry (FAI), Sheriff James Williamson concluded that there was no mechanism in place for reconciling it.
He stated: “The failure of Police Scotland to properly risk assess the call handling procedures and have a system of reconciliation was an organisational failure.”
Police Scotland expressed their regret to the relatives of the deceased once more while also stating that call handling systems have undergone “significant improvements” since 2015 and are now “incomparable” to those that were in use at the time.
